Detailed list of each problem area/obstacle identified. Includes any significant evaluations or assessments made during the group discussion.

V. Action Items/Decisions

Detailed list of all action items/decisions generated to resolve the problem areas.

VI. Lessons Learned

Detailed list of lessons learned (both positive and constructive) from the project based on group discussion and evaluation
